In preparation for the procedure, the shoulder is cleansed and sanitized. Local anesthetic is administered to numb the tissue at the injection site. The physician may choose from multiple needle approaches, depending on which part of the shoulder requires treatment.

The physician carefully guides a needle into the shoulder joint. The physician may guide the needle with the help of an x-ray device called a fluoroscope. If so, the physician may inject contrast dye to confirm the needle’s placement.

When the needle is in position, the physician injects a mixture of anesthetic and steroid medication. The medication will help reduce inflammation and relieve pain.

When the injection is complete, the needle is slowly withdrawn. The injection site may be covered with a small bandage. For many patients, extended pain relief begins within two to three days of the injection. In some cases it may be necessary to repeat the procedure one or more times to receive the full benefit of the treatment.
The shoulder is the site of multiple injuries and inflammatory conditions that lend themselves to diagnostic and therapeutic injection. Joint injection should be considered after other therapeutic interventions such as nonsteroidal anti-inflammatory drugs, physical therapy, and activity-modification have been tried. Indications for glenohumeral joint injection include osteoarthritis, adhesive capsulitis, and rheumatoid arthritis.

For the acromioclavicular joint, injection may be used for diagnosis and treatment of osteoarthritis and distal clavicular osteolysis. Subacromial injections are useful for a range of conditions including adhesive capsulitis, sub-deltoid bursitis, impingement syndrome, and rotator cuff tendinosis. Scapulothoracic injections are reserved for inflammation of the involved bursa. Persistent pain related to inflammatory conditions of the long head of the biceps responds well to injection in the region. The proper technique, choice and quantity of pharmaceuticals, and appropriate follow-up are essential for effective outcomes.

Post-Injection Procedure

The injection itself only takes a few minutes, but the overall procedure will usually take between thirty and sixty minutes. After the joint injection procedure, the patient typically remains resting on the table for twenty to thirty minutes, and then is asked to move the area of usual discomfort to try to provoke the usual pain. Patients may or may not obtain pain relief in the first few hours after the injection, depending upon whether or not the joint that was injected is the main source of the patient’s pain. On occasion, the patient may feel numb or experience a slightly weak or odd feeling in the leg for a few hours after the injection. The patient will discuss with the doctor any immediate relief of pain, and will then record the levels of pain relief during the next week. A pain diary is helpful to clearly inform the treating physician of the injection results and in planning future tests and/or pain management treatment, as needed.
